Requirements
  • Master's or Doctorate degree in social work from an accredited program
  • 4-6 years of work experience
  • Current South Carolina Licensed Independent Social Worker license (LISW)
Responsibilities
  • Provide comprehensive psychosocial assessment, counseling, crisis intervention, and resource navigation for adults with serious illness and their families.
  • Facilitate goals-of-care conversations and advance care planning.
  • Provide caregiver support and connect patients to community resources such as hospice.
  • Help reduce avoidable emergency department visits and improve transitions of care.
  • Support delivery of goal-concordant, whole-person care.
  • Ensure alignment with national palliative care guidelines.
  • Relieve clinicians of tasks within the social work scope.
  • Support strategic initiatives including transitions clinics and oncology integration.
  • Strengthen MUSC's ability to improve patient experience, reduce total costs of care, and address social determinants of health in the ambulatory setting.
Desired Qualifications
