Requirements

Responsibilities
  • Provides direct patient care as described in state regulations for RN, in all cases involving nursing services.
  • Assists in the implementation of changes in the Plan of Care for the improvement of client care.
  • Makes the initial evaluation visit as applicable and re-evaluates the patient’s needs on a regular basis.
  • Provides nursing services, treatments and diagnostic procedures requiring specialized skills and assuming full responsibility for each assigned patient.
  • Participates in in-service programs, as well as training, and teaching other nursing personnel as required.
  • Promotes and maintains client health through teaching self-care and rehabilitative measures.
  • Develops and implements the treatment plan for each patient under the direction of their physician and plan of care.
  • Observes and reports symptoms, reactions to treatments, drugs, and changes in the patient’s physical and/or emotional condition.
  • Initiates preventative and rehabilitative nursing procedures as appropriate for the patient’s care and safety.
  • Maintains clinical and progress notes for each assigned patient receiving care and providing progress reports to their physician.
  • Coordinates services for patients, as needed, to other agencies and health care providers as appropriate.
  • Provides supervision of Aide/Certified Nursing Assistant services and prepares written instructions for care provided by Aide/CNA.
  • Considers age-related factors with patient care for the adult and elderly when providing home care.
  • Maintains accurate time records.
  • Submits required documentation in a timely manner.
  • Assists State Administrator and/or Regional Administrator in decision making and policy formation as it pertains to the Home Health Program.
  • Participates in surveys, studies and special projects as assigned.
  • Other duties as assigned.
