Requirements
  • Legally authorized to work in the United States of America.
  • An active, unrestricted clinical certification or licensure is required, such as Patient Care Technician, Certified Nursing Assistant, Medical Assistant or Certified Medical Assistant, Licensed Practical Nurse, Registered Nurse, or a comparable clinical credential.
  • A professional, compassionate bedside manner with a patient-centered approach and the ability to build rapport in a clinical setting is required.
  • Strong working knowledge of healthcare terminology and care coordination principles is required.
  • Excellent organizational and time-management skills with the ability to manage multiple priorities effectively are required.
  • Professional and proactive communication skills across Microsoft Teams, email, phone, and in-person interactions are required.
  • The ability to cultivate and maintain positive, collaborative relationships with patients, families, providers, and facility staff is required.
  • Proficiency in Microsoft 365, including Outlook, Word, and Excel, is required.
  • The ability to work full-time Monday through Friday, 6:00 a.m. to 2:00 p.m. Central Standard Time in an on-site role based in the Jackson, Mississippi area is required.
  • A valid, unencumbered driver's license, consistent reliable transportation, and current auto insurance with proof of coverage are required.
  • The ability to travel between facilities as required is necessary.
  • Successful completion of a background check is required.
Responsibilities
  • Provide consistent, on-site engagement with dialysis patients to support care plans, follow-ups, education, and care transitions.
  • Coordinate and align patient care using the electronic medical record to ensure continuity, quality outcomes, and timely follow-up.
  • Schedule patient visits for providers and support daily clinic workflow based on patient attendance and clinical priorities.
  • Collaborate with Ennoble Care providers, dialysis facility staff, and a multidisciplinary care team to support integrated, patient-centered care.
  • Serve as a liaison between providers, patients, families, and facility staff, ensuring clear, proactive communication.
  • Communicate patient needs, changes in condition, safety concerns, and urgent matters promptly to the appropriate clinical leader.
  • Identify and resolve barriers impacting patient care coordination and the overall patient experience.
  • Maintain accurate, timely documentation in accordance with organizational standards, workflows, and healthcare regulations.
  • Manage multiple priorities in a fast-paced clinical environment while demonstrating professionalism, empathy, and sound judgment.
  • Contribute to team collaboration, daily operations, and facility partnerships while maintaining organizational and technical proficiency.
Desired Qualifications
  • A dialysis-specific certification such as CDT, CHT, CCHT, or CCHT-A is highly preferred.
  • Experience managing provider schedules and coordinating patient appointments within a clinical setting is highly preferred.
  • At least one year of experience working directly with hemodialysis or nephrology patients in a dialysis facility is highly preferred.
