  • Ability to build and maintain positive working relationships with individuals and work groups through open communication and collaboration, and work with others to accomplish goals and objectives.
  • Proficiency in Microsoft Office Suite.
  • A high school diploma or GED is required.
  • One year of related experience is required.
Responsibilities
  • Communicate with caregivers and families of individuals with dementia.
  • Provide resources, coaching, encouragement, and active listening.
  • Conduct home safety assessments.
  • Make outbound phone calls and electronic communications.
  • Complete occasional home visits.
  • Present case studies from the patient's perspective to the primary care team to facilitate a patient-centered practice.
  • Perform outreach calls, engage in face-to-face encounters, and perform home visits or other community-based visits when necessary.
  • Communicate relevant information to providers in a timely manner and document appropriately in the electronic health record.
  • Participate in local community engagement activities with local agencies as appropriate.
  • Counsel patients with appropriate and relevant educational materials.
  • Identify patterns of concern that surpass neighborhood capacity for health and engage diverse outside resources accordingly.
  • Enhance patients' individual and community capacity by growing health knowledge and self-sufficiency through outreach, informal counseling, social support, and advocacy.
  • Assist patients in connecting to appropriate health care information and services, addressing barriers to receiving follow-up services and treatment.
  • Provide patient reminders about upcoming appointments and next steps of treatment.
  • Serve as a liaison between patients, health and social services, and the community to enable access and improve the quality and competence of service delivery.
  • Perform other duties as assigned.
