As the Clinical Manager, you will effectively manage clinical operations, services, personnel performance, and office management on an ongoing basis. Responsibilities
- Ensure that the client’s plan of care is individualized and appropriate, executed as written and reassessed by the appropriate health care professional when there is a significant health status change in the client’s condition, at the physician’s request and after hospital discharge. Ensure appropriate documentation is completed for all patients transferred to an inpatient facility.
- Hold weekly Case Conferences to discuss client issues. Determine if clients that are planned for discharge meet the requirements for Management and Evaluation or any other skilled service.
- Develop corrective plans congruent with the SOP and Policies and Procedures. Route the corrective plan to the Administrator and Vice President.
- Recruiting, hiring, orientating and retaining of field and office staff in order to cover referrals and patient visit needs. Conduct general and job-specific orientation.
- Supervise the day-to-day office and clinical operations.
Qualifications
- Graduate of an accredited school of nursing. Bachelor’s degree preferred, with a minimum of 3+ years of home health care experience.
- Must have a minimum of 2+ years of nursing experience within the last 36 months.
- Medicare home health experience required.
- Current licensure in State, CPR certification.
- Must have a good understanding of the Federal, State and local laws/regulatory guidelines governing the operation of a Home Health Mission Healthcare.
- Have valid driver’s license with no restrictions and auto insurance meeting state’s insurance coverage minimums.
- Supervisory experience not required but preferred.
