Requirements
  • Current CPR certification.
  • A valid California driver's license.
  • Eligibility to be insured under the organization's liability policy with a clean Department of Motor Vehicles report.
  • A personal vehicle and proof of current auto insurance.
  • Successful completion of a pre-employment background check and fingerprinting compliance.
  • Effective written and verbal communication in English with colleagues, supervisors, and participants.
  • Ability to conduct face-to-face client visits in the community and work in an office environment.
  • Knowledge of the basic problems and needs of people diagnosed with a physical disability or emotional disturbance.
  • Ability to assess and prioritize multiple tasks, projects, and demands.
  • Ability to work within deadlines to complete projects and assignments.
  • Ability to establish and maintain effective working relationships with coworkers and community providers.
  • Ability to use a computer and standard software, including an electronic medical record.
  • Ability to work independently and as a team member.
  • Ability to sit, stand, bend, lift, walk, and move intermittently during working hours.
  • Ability to lift up to 35 pounds without assistance.
  • Ability to drive intermittently.
Responsibilities
  • Work with a care management team that provides comprehensive case management services.
  • Assist the team in finding community resources to meet participants' needs, including housing, medical equipment and supplies, primary care physicians and specialists, food, clothing, household goods, and medical transportation.
  • Provide guidance to participants through application processes for health benefits and insurance coverage, In-Home Supportive Services, CalFresh, waiver programs, and managed care programs.
  • Engage in advocacy efforts to ensure participants receive timely medical services and remain safe in their community.
  • Educate participants so they can better understand service benefits and community support programs.
  • Document consumer interactions and maintain timely reporting and documentation requirements.
  • Observe participants' behavior, report significant changes to a supervisor and medical provider, and document reports promptly and appropriately.
Desired Qualifications
  • Experience with the justice-involved population.
  • Knowledge and experience with the Cal-AIM program, HUD, and housing programs.
  • Experience providing care management services and knowledge of low-income housing resources.
  • Knowledge of community and supportive services.
  • Bilingual skills, especially Russian, Spanish, Hmong, or Portuguese.
  • Education in social work or related fields such as gerontology, sociology, or psychology.
