- Hold Basic Life Support HealthCare Provider certification from the American Heart Association.
- Hold the applicable registered professional nurse license for the state of work location, including a compact-state license where applicable.
- Provide a driving record Motor Vehicle Report.
- Hold a state-issued driver license.
- Coordinate, facilitate, and perform interdisciplinary plans of care through assessment and evaluation of patient condition and care effectiveness under the direction of the attending physician.
- Travel to patient homes, Community-Based Residential Facilities, assisted living locations, hospitals, or other locations to meet patients and families or caregivers and assess care needs and acuity.
- Assess and coordinate patients’ discharge-planning needs with members of the healthcare team.
- Perform point-of-care testing according to policies and procedures when required.
- Collaborate, consult, and communicate with physicians regarding plans of care and changes in patient status; provide regular progress updates, recommend plan-of-care revisions, and document status and communications in accordance with regulations.
- Coordinate and communicate with multidisciplinary team members and outside agencies to achieve expected patient-care and education outcomes through end of life or discharge, ensuring accurate and complete documentation and coordination of unique care plans.
- Complete admission paperwork, verify insurance information, and complete comprehensive medical assessments by entering data into the clinical computer program.
- Provide skilled nursing care within the established plan of care, including taking vital signs, wound care, intravenous care, ostomy care, blood draws, injections, and physical assessment of the patient’s overall medical condition.
- Document visit notes, treatments, and care; follow the visit pattern according to physician orders and ensure required visits are completed accurately, completely, and on time.
- Collaborate with patients, families, and the healthcare team to develop comprehensive plans of care based on individual patient needs.
- Respond to calls from patients, family members, and others and complete follow-up to address needs, issues, or concerns.
- Work in a constant state of alertness and in a safe manner.
- Perform other duties as assigned.
