Requirements
  • Master’s Degree in Social Work (MSW) from an accredited university is required.
  • Current BLS certificate is required.
  • Prior experience in case management, social advocacy, and/or mental health preferred.
  • Bilingual: Spanish/English, Chinese/English or another second language is strongly preferred.
Responsibilities
  • Provide a psychosocial perspective to the interdisciplinary evaluation, assessment, plan of care, ongoing services, and disenrollment processes of the PACE program.
  • Conduct individual participant and/or family contacts; collateral contacts; participant and family education, assessment, and counseling; mobilization of resources; identification and management of behavioral health needs; case management; advocacy; and discharge planning.
  • Collaborate as part of the Interdisciplinary Team to optimize health status and quality of life of the PACE participants.
  • Support coverage needs at the various PACE centers across LA and Orange Counties, including unexpected or unplanned MSW absences.
  • Assist Social Work Manager with preparation, planning, and implementation for the Social Work department.
  • Engage in data collection and analysis, building reports, preparing presentations, and sharing results with PACE leaders.
  • Participate in numerous process improvement projects and ongoing committees to enhance psychosocial and behavioral health service delivery to PACE participants.
