Requirements
  • A bachelor's degree is preferred but not required, or two years of experience working with the behavioral health population.
  • Working knowledge of computer programs including email, Microsoft Word, Excel, PowerPoint, and the Windows operating system.
Responsibilities
  • Assist agency consumers with accessing medical, psychological, substance abuse, vocational, food, clothing, and other community services through referral-network development, referrals, and follow-up.
  • Provide case management to an assigned caseload.
  • Develop and monitor individual support plans in collaboration with agency consumers.
  • Assist agency consumers with problem solving and crisis intervention.
  • Accompany agency consumers to medical, psychiatric, and public benefit appointments as necessary.
  • Act as an advocate for consumers and encourage and support self-advocacy.
  • Identify problems in daily living skills and provide hands-on assistance or referrals to address them.
  • Support Facilities Management staff in maintaining apartment standards outlined by S:US and the Office of Mental Health.
  • Provide supportive counseling to agency consumers as needed.
  • Conduct regular home visits and hospital visits as necessary.
  • Maintain strong working relationships with therapeutic support services to ensure quality of care.
  • Document all services in compliance with agency standards.
  • Assist consumers and staff with moves, including lifting, packing, and cleaning.
  • Work as a member of the social service team to develop off-site recreation, socialization, educational, and skill-building activities supporting community inclusion and integration.
  • Serve as a liaison to community-based services and groups, including civic, social service, government, and business organizations.
  • Participate in individual and group supervision, team meetings, case conferences, division meetings, and trainings.
  • Adhere to the on-call cell phone system to facilitate crisis intervention, emergencies, and other situations requiring immediate action.
  • Perform other duties as assigned.
Desired Qualifications
  • Two years of experience with individuals with mental illness, substance use issues, and/or a history of homelessness.
  • Experience in group work, tenant organization, case management, referral, and/or community outreach.
  • Experience in a supportive housing setting.
  • Bilingual English/Spanish speaking ability.
