Requirements

Responsibilities
  • Triage new patient appointments to the appropriate providers and assure timely scheduling of initial appointments.
  • Review outside medical records for appropriate scheduling.
  • Assess barriers to care and refer patients to support services and local or national organizations when needed.
  • Educate patients about treatment plans based on diagnosis.
  • Support patients throughout the care continuum.
  • Counsel individuals and patients on positive health practices.
  • Collaborate with multidisciplinary teams to outline the best treatment for patients.
  • Perform holistic evaluations of specialty populations using enhanced proven techniques and procedures.
  • Implement improvements to patient care, healthcare policies, and resources.
  • Mentor healthcare professionals by functioning as a preceptor or coordinating preceptors for visiting professionals, students, new graduates, and orientées.
  • Maintain professional knowledge through professional and technical organizations and continuing education programs, conferences, seminars, and workshops.
  • Adhere to University and unit-level policies and procedures and safeguard University assets.
  • Serve as the primary clinical intake point for complex plastic surgery referrals, including breast reconstruction, microvascular surgery, wound reconstruction, craniofacial surgery, and complex general reconstruction.
  • Conduct pre-visit intake by reviewing outside records, imaging, pathology, and operative reports.
  • Triage referrals to the appropriate surgeon or advanced practice provider based on diagnosis, acuity, and service line.
  • Identify patients appropriate for advanced practice provider-first pathways or multidisciplinary evaluation.
  • Provide patient education regarding procedure expectations, timelines, and recovery.
  • Coordinate postoperative follow-up, wound care, drain-management education, and escalation pathways for complications.
  • Manage EPIC in-basket messages, patient calls, and clinical coordination requests related to surgical care.
  • Enter and track orders, referrals, and follow-up requests to support order-based scheduling.
  • Coordinate care across multidisciplinary teams.
  • Serve as a liaison between patients, providers, and clinical or administrative care teams to ensure clear communication and alignment of care plans.
  • Provide emotional support and counseling throughout the surgical journey.
  • Serve as a clinical resource during implementation of new workflows, programs, or service-line expansions.
  • Collaborate with faculty, advanced practice providers, residents, and administrative leaders to support division initiatives.
