Requirements
- Must be a registered nurse
- Bachelor's degree
- Home health experience
- Recent psychiatric experience in an acute hospital or in home health
- 2-5 years of experience in psychiatric home health or related field
Responsibilities
- Works under the supervision of the Director of Operations and Case Manager and provides comprehensive case management to patients and their families
- Develops, implements, coordinates and evaluates patient care plans according to physician orders as allowed by the scope of practice
