Requirements
  • BLS Provider certification from the American Heart Association or American Red Cross must be obtained within 30 days of hire or job transfer.
  • A Registered Nurse credential from the Wisconsin Board of Nursing, or a current home-state license if the candidate qualifies under a multistate or compact-state license, is required before hire or job transfer.
  • A diploma from an accredited school or college of nursing, or required professional licensure at the time of hire, is required.
Responsibilities
  • Act as the central care coordinator, ensuring seamless management and interdisciplinary collaboration with physicians, specialists, therapists, social workers, and ancillary staff for optimal patient outcomes.
  • Implement, continuously monitor, and dynamically adjust the prescribed patient care plan based on clinical findings, patient progress, and team input.
  • Interpret, document, and execute authorized physician and nursing orders, including medications, tests, treatments, and procedures.
  • Continuously assess patients, document findings and treatments in the electronic health record, and immediately communicate significant changes to the physician and healthcare team.
  • Assess discharge needs from admission, collaborate with patients, families, social work, case management, and physicians to arrange post-acute resources, and provide education for a safe transition of care.
