Requirements
  • Previous experience in venipuncture.
  • Professionalism, integrity, accountability, confidentiality, care, and compassion in providing health services.
  • Ability to solve problems in a changing environment and work independently or in a team with minimal supervision.
  • Comfort using mobile devices for charting and HIPAA-secure messaging applications for care coordination.
  • Strong written and verbal communication skills.
  • Reliable personal transportation for travel to patients' homes.
  • Ability to access and navigate patient homes, including climbing stairs and carrying up to 20 pounds while walking.
  • Ability to pass national background, Department of Motor Vehicles, and valid clinical license checks.
  • State-required licensure and/or proof of training, or national certification where state-level licensure is not required.
  • Basic Life Support certification.
  • Proof of completion of a course, program, or certification based on licensure level.
  • Valid driver's license.
  • Current vehicle insurance.
  • Proof of vaccination or immunity for measles, mumps, and rubella; hepatitis B; varicella; tuberculosis testing within the past year; influenza or a signed exemption; and tetanus, diphtheria, and pertussis vaccination within the past 10 years.
Responsibilities
  • Ensure all required supplies and personal protective equipment are available for the day's patients.
  • Use mobile technology to access the daily schedule and travel to each patient's home.
  • Build welcoming connections with patients while collecting relevant information and performing services.
  • Provide low-acuity preventative and diagnostic services in patients' homes, including phlebotomy, specimen collection, diabetic foot screenings, retina examinations, and electrocardiograms.
  • Label collected samples properly and coordinate their delivery to the appropriate laboratory or shipment facility.
  • Perform services comfortably and conveniently for patients.
  • Use medical expertise and complete training to provide additional preventative care and diagnostic services, including diabetic retina imaging, electrocardiograms, and digital-stethoscope recordings.

It coordinates routing, inventory, diagnostic device integration, and specimen drop-offs to make visits accurate and efficient. After each visit, it reconnects patients with their primary care physicians by delivering encounter summaries to inform follow-up care, and it offers tools to sustain care continuity, in-home screening, and staffing support. Its goal is to close care gaps, improve access to preventive care, and reduce pressure on healthcare systems by delivering coordinated, person-centered care.
