  • Competence in maintaining patient confidentiality and managing sensitive information.

Responsibilities
  • Evaluate patients' medication regimens and identify potential adherence barriers.
  • Create and implement tailored adherence strategies using reminders and tools.
  • Conduct regular follow-ups to track adherence and adjust plans as needed.
  • Work with physicians and pharmacists to ensure coordinated care and communication.
  • Recognize and assist in overcoming barriers such as financial issues and side effects.
  • Maintain accurate records, analyze data, and report on adherence metrics.
  • Ensure patients receive necessary support and access to resources.
  • Keep current with best practices and train other healthcare staff on adherence strategies.
  • Other duties as assigned.
Desired Qualifications
  • High School diploma/GED, advanced degrees or certifications in relevant fields (e.g., Certified Pharmacy Technician, Certified Health Education Specialist) are preferred but not required.
  • 1-3 years’ experience as a Pharmacy Technician, Medical Assistant, Medical Records Technician, or Referral Coordinator.
  • 1-3 years’ experience working with medical records and/or patients in a healthcare setting.
