Requirements
  • Must possess a current, unencumbered license to practice as a Licensed Practical Nurse in New Jersey.
  • Graduate of an accredited school of nursing.
  • Proficiency in providing care for patients diagnosed with a Mental Health illness is essential.
  • Adheres to REMS protocols for Esketamine.
  • Training in de-escalation techniques and intervention required within 1 month of employment.
  • Demonstrates compliance with the CH ASPIRE Standards of Performance.
Responsibilities
  • Provide direct patient care and support to patients in the Outpatient Mental Health Counseling Center, including Spravato (Esketamine) program and Outpatient Substance Use Disorder services, under the supervision of registered nurses, advanced practice providers and physicians.
  • Monitor vital signs and provide support during treatment sessions and ensure adherence to safety protocols.
  • Educate patients regarding their treatments and treatment options and ensure a therapeutic and safe environment.
  • Maintain accurate medical records and communicate and collaborate effectively with the multidisciplinary team; ensure adherence to safety and all Organizational, State and Federal regulatory standards.
  • Sign and date all entries made in the patient's medical record and chart nurses' notes in an informative and descriptive manner reflecting the care provided and the patient response to care.
  • Prepare and administer medications as ordered by the physician and within the guidelines of good nursing practice; ensure discontinued drugs and narcotics are returned to the Nursing Supervisor/Unit Manager or provider pharmacy for destruction or credit.
  • Review medication orders for completeness, accuracy, transcription, and adherence to stop policies.
  • Perform daily rounds of assigned patients in accordance with acceptable nursing standards.
  • Transcribe physician's orders/authorized orders per policy.
  • Support patients’ well-being through prevention and education, providing health promotion and maintenance.
  • Conduct intake screening as needed, evaluation and triage.
  • Proficient in understanding Mental Health diagnoses and nursing care planning.
  • Report discrepancies concerning physician's orders or charting errors to the Director of Nursing or Supervising Provider.
  • Complete Transfer Forms and accident/incident forms as required; submit to Nurse Manager.
  • Competent to care for adult and geriatric (65+) patients; complete all documentation as required.
  • Participate in maintenance of the facility's quality assurance program for nursing services, including restraint reduction.
  • Adhere to department policies and procedures and interpret them to personnel, patients, visitors and government agencies as required.
  • Admit, transfer, and discharge patients as per policy; maintain accident/incident reporting.
  • Record vital signs and other nursing measurements as necessary (e.g., TPR, blood pressures).
  • Participate in infection control program to monitor communicable diseases and maintain universal precautions.
  • Maintain patient rights and confidentiality of patient information.
  • Ensure fair, compassionate care and privacy for all patients; respond to complaints and grievances with appropriate reporting.
  • Ensure that department personnel follow policies and dress code and safety regulations.
  • Review patient care plans; communicate changes to Director of Nursing or Supervising Provider and ensure care plan is followed.
  • Participate in reporting hazardous conditions or equipment and ensure maintenance of safety protocols.
  • Order prescribed medications, supplies, and equipment as necessary; ensure adequate stock and escalate needs to Director of Nursing or Supervising Provider.
  • Ensure narcotic records are accurate and report discrepancies to Nursing Supervisor/Unit Manager.
  • Ensure that department policies and procedures are followed by all nursing personnel on shift.
  • Provide leadership to nursing personnel on unit/shift and maintain a warm, calm environment.
Desired Qualifications
  • Previous experience in a mental health or related field preferred.
