Requirements
  • A Bachelor's Degree.
  • A minimum of 3-5 years of experience in healthcare quality, population health, or value-based care programs.
  • Experience in an integrated delivery system or large physician enterprise.
  • Familiarity with electronic medical record reporting tools, including Epic.
  • Demonstrated understanding of Hierarchical Condition Categories coding.
Responsibilities
  • Support implementation and ongoing management of bundled payment programs across targeted service lines.
  • Monitor performance metrics, including cost, utilization, and quality outcomes.
  • Work with clinical and operational teams to align workflows with bundle requirements and improve episode performance.
  • Support quality-based contract negotiations.
  • Track performance against shared savings contracts, including Medicare and commercial value-based arrangements.
  • Partner with operations, finance, and analytics teams to identify drivers of cost and quality performance.
  • Translate contract quality metrics into actionable plans for practices and providers.
  • Identify risks to performance and support mitigation strategies.
  • Work with coding and clinical teams to appropriately communicate patient risk levels using Hierarchical Condition Categories risk-coding guidelines.
  • Lead initiatives to improve quality performance, including Healthcare Effectiveness Data and Information Set and Star Ratings measures.
  • Support practice-level strategies for clinical gap closure, including preventive screenings, chronic disease management, and annual wellness visits.
  • Collaborate with care teams and outreach functions to improve patient engagement and completion of care gaps.
  • Analyze performance data to identify trends, variation, and opportunities for improvement.
  • Prepare structured reports and executive-level summaries highlighting key insights and recommended actions.
  • Support internal and external reporting requirements related to quality and value-based contracts.
  • Serve as a liaison between quality, operations, and clinical teams.
  • Provide education and guidance on quality measures, documentation, and workflow expectations.
  • Participate in performance reviews with practices and contribute to action planning.
  • Connect with patients as needed to support continuity-of-care measures and flag and support high-risk patients for clinical intervention.
  • Identify opportunities to standardize quality workflows across practices.
  • Analyze and make recommendations for new program development, and provide implementation support for new programs based on a data-driven strategy.
  • Support development and implementation of standard operating procedures related to gap-closure workflows, bundle program operations, and documentation and coding practices.
  • Collaborate with information technology and electronic medical record teams to optimize systems supporting quality measurement and reporting.
  • Work with leadership teams throughout the health system to help reduce costs, enhance revenues, achieve effective utilization and quality goals, and analyze and use information to support management decisions.
Desired Qualifications
  • A Master's Degree in Business Administration, Health Administration, or a related field is preferred.
  • Lean or Six Sigma certification is preferred.
  • Healthcare coaching certification or experience is preferred.
