- A Licensed Clinical Social Worker (LCSW) credential is required.
- Two to four years of social work experience within the last five years is required; Master of Social Work fieldwork is not included.
- Provide telephonic psychosocial assessments and interventions to assist patients and caregivers with care transitions and coping with illness, diagnosis, treatment, and life situations, including emotional, mental, and substance abuse disorders.
- Educate members, family members, and caregivers about adapting to a member’s diagnosis, illness, treatment, or life situation.
- Address biopsychosocial stressors related to disease management to improve members’ overall well-being and reduce emergency department utilization and readmissions.
- Address barriers related to social determinants of health and connect members with community programs.
- Coordinate and collaborate with hospitals, skilled nursing facilities, home health, hospice, and other community agencies to advocate for members’ ongoing care needs.
- Participate in interdisciplinary team meetings to discuss members’ needs and address care gaps.
- Assist with advance care planning and completion of advance directives and Physician Orders for Life-Sustaining Treatment.
- Coordinate palliative care and end-of-life services when appropriate.
- Experience in a managed care environment is preferred.
- Hospital experience is preferred.
