Requirements
  • NY State licensed Registered Nurse required
  • Current CPR certification required
  • Ability to drive is necessary to perform his/her/their job responsibilities
  • Excellent oral/written communication and organizational skills
Responsibilities
  • Ensures medical care for the individuals within a designated cluster
  • Identifies and develops an individualized plan for nursing services for any individual who requires nursing care
  • Ensures all medical services including testing are completed at the frequency recommended by the provider
  • Makes weekly visits to the residence for individual medical monitoring and review of all medical reports
  • Tracks medical appointments using the agency tracking forms for each site in the residential cluster
  • Attends monthly support meetings with AD Nursing to review health status of individuals
  • Provides initial and ongoing supervision/training/support to unlicensed direct care staff
  • Conducts annual clinical performance evaluations for unlicensed direct care staff
  • Monitors and visits individuals who are hospitalized for medical or psychiatric reasons
  • Participates in a 24-hour, 7 days a week on-call system
  • Ensures back-up nursing coverage during use of accrued agency time
  • Responsible for providing supervisor with a daily nursing report regarding any change in individual’s health status
  • Ensures pharmacy reviews are completed every 6 months for IRAs and quarterly for ICFs
  • Leads team meetings for an individual prior to any scheduled surgery
  • Ensures Compass and State audits are prioritized
  • Contacts family members to secure consent for medical procedures
  • Coordinates and presents medical information necessary for Surrogate Decision Making Committee review
  • In the event the RN is unable to perform any part of her job responsibility, she/he is required to notify their supervisor immediately
  • Requests time off as per the agency handbook
  • Is able to drive, as is necessary to perform his/her residential nurse job responsibilities
  • Performs other duties as requested by supervisor
  • This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services
  • Ensures that work-related safety procedures are in place
  • Reports to Justice Center as a mandated reporter if there is reasonable cause to suspect abuse or neglect
  • Adheres to all regulations, agency policy and procedures
  • Arrives to work by start of scheduled shift

Applications Open: Statewide Community Of Practice On Dei Focusing On Developmental Disabilities

BROOKVILLE, N.Y., April 30, 2024 /PRNewswire/ -- A coalition of New York disability agencies is seeking volunteer leaders for a statewide Community of Practice (CoP) advisory team on diversity, equity and inclusion (DEI) to improve service outcomes for New Yorkers with intellectual and developmental disabilities (I/DD). Applications are now open through Sunday, June 30, at 11:59 p.m.The New York State Council on Developmental Disabilities (CDD) tapped AHRC Nassau, a disability services provider based on Long Island, to lead this "participatory learning process" with a five-year grant of $750,000. The coalition currently includes WRI Solutions, the New York Alliance for Inclusion and Innovation, and the National Alliance for Direct Support Professionals (NADSP).Volunteers participating in the CoP will focus on DEI strategy development and implementation tailored to the I/DD community. Their goal will be to elevate care quality and establish DEI best practices individually, within service organizations, and across New York State through trainings and policy development.Together, the coalition will develop a minimum of five regional teams totaling approximately 250 stakeholders.
