Requirements
  • A master's degree from an accredited school of social work and three years of relevant experience, or an equivalent combination of education and experience.
  • Licensed Medical Social Worker state licensure within 18 months is required.
  • Licensure as a social worker, as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services, is required. If licensure has not been obtained upon employment, it must be obtained within 18 months, unless duties are performed under the Office of Mental Health, in which case a limited permit of no more than one year or New York State licensure is required upon employment start.
Responsibilities
  • Serve as Team Leader for the General Ambulatory service and primary therapist for adults with severe and persistent mental illness.
  • Provide supervision to an interdisciplinary group of clinical staff, as appropriate.
  • Collaborate in policy and program development, operations management, and program activity planning.
  • Perform screening, diagnostic formulation, evaluation, and psychosocial assessments of patients and their families.
  • Provide social work and behavioral health treatment and education to patients and families through individual, family, and group therapy.
  • Coordinate comprehensive treatment and discharge planning for assigned patients.
  • Participate in an interdisciplinary treatment team through consultation, psychosocial diagnosis, treatment planning, and periodic care review.
  • Assist with clinical supervision and contribute to the evaluation of interdisciplinary staff.
  • Assist with onboarding new therapists, including policies, procedures, clinical teaching, and orientation.
  • Provide supervision to students and interns, as appropriate.
  • Manage interdisciplinary treatment-team meetings, facilitate clinical-case discussions, and document meeting minutes.
  • Collaborate with collateral contacts and link patients to community resources.
  • Coordinate with community and hospital resources to ensure continuity of care.
  • Document treatment, interventions, and contacts in the patient's electronic medical record in compliance with applicable policies.
  • Participate in cultural-competency events and training appropriate to job duties.
  • Attend Social Work Division, Social Work in Psychiatry, and program staff meetings.
  • Ensure implementation of policies and procedures related to professional practice, discharge planning, and patient care.
  • Complete time reporting, hospital or division forms, mandatory in-service training, and required reviews.
  • Participate in the Social Work Division committee process and facilitate quality-assurance and staff-development activities.
  • Provide coverage and accessibility for intakes, vacations, and sick calls as needed.
  • Supervise social work or other master's-level graduate students based on experience and supervisor direction.
  • Offer learning experiences for psychiatric residents, medical students, and family-medicine residents when invited.
  • Participate in interdisciplinary research based on experience and interest.
