- A high school diploma or General Educational Development (GED) certificate is required.
- The candidate must have completed 105 hours Certified Nursing Assistant Program as approved by the Indiana State Board of Health or equivalent as approved by the Indiana State Board of Health.
- The candidate's name must be placed on the Certified Nursing Assistant Registry at the time of hire as required by the Indiana State Board of Health, and the individual must be in good standing and maintain certification throughout employment.
- Assist with admission, transfer and discharge of residents as instructed by the Charge Nurse.
- Assist residents with tending to personal care and activities of daily living.
- Provide direct patient care as assigned by the Charge Nurse.
- Provide proper documentation of resident care.
- Promptly communicate with Charge Nurse and leadership team with any changes in the resident’s condition.
