Requirements
  • High School Diploma or equivalent (GED) with a minimum of three years of work experience in a data entry/administrative position; or an associate degree with two years of work experience in a data entry/administrative position: or a bachelor’s degree with one year of work experience in a data entry/administrative position.
  • Ability to establish and maintain cooperative working relationships with a wide range of groups and individuals.
  • Extensive knowledge of office management, policies and practices in a complex environment required.
  • Ability to establish cooperative working relationships with diverse groups and individuals, medical staff, and other health care disciplines and understanding of the consultative process.
  • A Bachelor's degree in nursing and three years’ experience as a registered nurse.
  • Current South Carolina licensure or compact state required.
  • Current Basic Life Support (BLS) required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.
  • Certified Clinical Transplant Coordinator (CCTC) is preferred.
Responsibilities
  • Provides administrative support to the transplant team throughout the transplant phases; pre-transplant, intra-transplant, waitlist and post-transplant. Obtain, coordinate, and maintain transplant patient records. Work in conjunction with patients and referring providers to assist patients in accessing transplant resources and care.
  • Actively participates in quality assurance activities.
  • Serves as liaison between the transplant team and outside agencies.
  • Performs other duties as assigned.
  • Coordinates the care of the transplant patient throughout the transplant phases; pre-transplant, intra-transplant, waitlist and post-transplant. Collaborates with transplant team members to provide the most cost-effective and appropriate care for the transplant populations. Ensures systematic approach for the plan of care in collaboration with other disciplines to meet patient and family needs.
  • Demonstrates ability to think critically, evaluate and complete the process of triaging medical concerns for transplant population from pre to post transplant phases. Demonstrates ability to complete tasks utilizing standards of care dictated by transplant regulations. Demonstrates ability to access EMR and Donor Net when applicable.
  • Initiates and/or actively participates in quality assurance activities. Evaluates patient population for trends and risk factors and recommends innovative strategies to improve quality of care. Favorably affects the cost, quality and outcomes of services. Negotiates effective practice approaches. Leads collaborative initiatives with all disciplines to identify systems and processes to improve patient outcomes.
  • Develops own professional role in collaboration with immediate manager, works towards obtaining CCTC two years from start date. Maintains membership and actively participate in at least one professional organization. Participates in institutional, local, and/or national committee. Remains current with literature as demonstrated by presentation(s), abstracts, and/or articles and incorporating new knowledge into clinical practice.
