Requirements
  • A Nursing Degree from an accredited college or university, or graduation from an approved Licensed Practical Nurse or Licensed Vocational Nurse program, is required.
  • A current, unencumbered, active license to practice as a Licensed Practical Nurse or Licensed Vocational Nurse in the state is required.
  • Current cardiopulmonary resuscitation certification is required.
  • All required continuing education and licensing must be maintained.
  • Good standing with the State Board of Nursing must be maintained at all times.

Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants in accordance with applicable rules, regulations, and facility guidelines.
  • Ensure that nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Review department policies, procedure manuals, and job descriptions, and recommend revisions.
  • Plan shift services, programs, and activities with assigned nursing staff and support personnel.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total care regimen.
  • Participate in the nursing department quality assurance program and facility inspections conducted by authorized government agencies.
  • Review and update resident discharge plans and assist with planning nursing services for discharge.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents; complete related forms, records, reports, and documentation.
  • Maintain census reports, resident charge slips, medical forms, evaluations, studies, and other administrative records.
  • Protect resident and facility information by maintaining confidentiality, safeguarding access credentials, and reporting unauthorized access or disclosure.
  • Complete and file records and charts upon resident admission, transfer, and discharge.
  • Receive telephone orders from physicians and record them on physician order forms.
  • Transcribe physician orders to resident charts, cardexes, medication cards, treatment plans, and care plans.
  • Document nursing care, resident responses, accidents, incidents, transfers, diet orders, and other required information in resident medical records.
  • Prepare and administer physician-ordered medications and treatments after verifying resident identity.
  • Maintain adequate supplies of medications, medical supplies, and equipment, and report supply needs.
  • Maintain accurate narcotic records, report medication discrepancies, review medication cards, and dispose of drugs and narcotics according to procedure.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend personnel dismissals or transfers to the Nurse Supervisor.
  • Report staffing shortages and absentee call-ins to the Nurse Supervisor.
  • Develop work assignments and provide leadership to nursing personnel assigned to the unit or shift.
  • Make daily rounds to ensure nursing personnel perform assignments according to nursing standards and report problem areas.
  • Ensure nursing assistants are enrolled in or have graduated from approved training programs and that nurse aide trainees are directly supervised by a licensed nurse.
  • Meet regularly with nursing personnel to identify and correct problem areas and improve services.
  • Maintain effective working relationships with interdepartmental personnel, residents, visitors, and support departments.
  • Receive and give nursing reports at the beginning and end of shift duty hours.
  • Report occupational exposures, suspected fraud, and information-security incidents according to facility procedures.
  • Inform nursing personnel about new admissions and prepare rooms for new residents.
  • Greet and orient newly admitted residents and family members to the facility.
  • Make physician rounds and arrange diagnostic and therapeutic services as ordered.
  • Consult with residents’ physicians regarding care, treatment, and rehabilitation.
  • Review resident charts for treatments, medications, diets, and other care requirements.
  • Implement nursing objectives and standards and verify that prescribed treatments are administered properly.
  • Provide or ensure licensed nursing care, certified nursing assistant care, and qualified nurse aide trainee care according to resident care plans and wishes.
  • Coordinate social and activity programs with nursing service schedules.
  • Notify attending physicians and next of kin about accidents, incidents, and changes in residents’ conditions.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Inspect treatment areas and resident care areas daily to ensure cleanliness and safety.
  • Perform catheterization, tube feedings, suctioning, dressing and bandage changes, colostomy and drainage-bag care, blood collection, massages, range-of-motion exercises, and care of dying residents as required.
  • Apply restraints according to facility policies and procedures.
  • Collect sputum, urine, and other specimens for laboratory testing and record temperatures, pulses, respirations, and blood pressures.
  • Monitor seriously ill residents and check residents who cannot call for help.
  • Verify that food brought by residents’ families or visitors complies with dietary allowances.
  • Meet with residents and family members and report concerns to the Nurse Supervisor.
  • Arrange transportation and escort discharged residents to the pickup area.
  • Notify family members of resident deaths, contact funeral homes when requested, and follow post-mortem procedures.
  • Develop, plan, conduct, and schedule in-service training classes for nursing personnel.
  • Orient new employees to the shift, policies, procedures, job position, and duties.
  • Assist with standardizing work methods and training personnel regarding blood and body-fluid exposure tasks.
  • Assist the Director in planning clinical supervision for nurse aide trainees.
  • Attend outside training, annual facility in-service programs, advance-directive training, and continuing education programs.
  • Monitor personnel compliance with safety regulations, dress codes, hand hygiene, infection control, and use of personal protective equipment.
  • Maintain clean and sanitary nursing work areas, resident care rooms, and treatment areas.
  • Participate in infection-control programs and ensure proper isolation precautions and disposal of protective equipment.
  • Ensure fire-safety and disaster-preparedness drills are conducted safely.
  • Ensure safe operation of nursing equipment, use of material safety data sheets, and maintenance of adequate medication and supply stock.
  • Review resident care plans daily and communicate required changes to the Nurse Supervisor.
  • Ensure nursing notes reflect care-plan implementation and that certified nursing assistants understand and follow resident care plans.
  • Assist with planning, scheduling, and revising the Minimum Data Set, including implementation of Resident Assessment Protocols and triggers.
  • Provide data to the Quality Assurance and Assessment Committee as requested.
