Responsibilities
  • Develop a comprehensive social history and assessment of each resident, including problems and strengths, special needs and preferences, and implications for the care plan.
  • Assist with the coordination of Health Center admissions and assessments.
  • Assist residents and families with transitions to new environments and orient them to the facility, services, limitations, and resident rights.
  • Participate in developing individual care plans, including goals and approaches for the Social Services component.
  • Attend scheduled care planning conferences, address issues involving schedules, treatment, and environment, and encourage resident and family participation when appropriate.
  • Ensure that advance directives are complete, maintained, and placed in resident charts.
  • Complete mandatory documentation in a timely manner in accordance with applicable regulations.
  • Maintain confidentiality of admission information and resident records in accordance with HIPAA and the community's Code of Ethics.
  • Maintain ongoing relationships with families to discuss needs or concerns and mediate issues between residents, families, and staff.
  • Coordinate discharges and transfers to ensure appropriateness and continuity of care, and develop comprehensive discharge plans with residents, families, and multidisciplinary staff.
  • Provide or identify referrals, agencies, and community resources to meet resident and family needs.
  • Coordinate support groups, participate in policy decisions affecting resident life issues, and advocate for resident rights.
