Requirements
  • Valid Personal Support Worker Certificate from a Ministry of Training, Colleges, and Universities or the Ministry of Education approved program
  • RN/RPN Student with minimum successful completion of year-one of the program
  • Internationally licensed Nurse, Midwife or Physician
  • Minimum 1 year experience with palliative patients
  • Current CPR and First Aid Certificate
  • Current Police check suitable for working with the vulnerable sector
  • Must possess the physical ability to perform the day-to-day duties described in the duties and responsibilities
  • G2/G License with access to a vehicle
Responsibilities
  • Assist palliative patients with personal care and activities of daily living.
  • Support patients, families and caregivers – enhancing quality of life, comfort, dignity and security in patients’ end of life journey under the direction of an Integrated Care Program Supervisor who is a nurse.
  • Provide specialized care as specified in the care plan, according to protocols, within scope of practice and in accordance with Spectrum Health Care’s policies and procedures.
  • Provide companionship and comfort, creating a calm and respectful environment tailored to the client's needs, preferences, and goals of care.
  • Assist with activities of daily living such as bathing, toileting, hygiene, skin care, light meal preparation, feeding and transfers.
  • Perform light housekeeping duties such as decluttering and cleaning.
  • Listen, observe and communicate any concerns/changes in the patient’s physical and/or emotional condition to the Integrated Care Program Supervisor.
  • Travel within assigned region between patients’ residences or facilities.
Desired Qualifications
  • Fundamentals of Hospice Palliative Care; or Core Concepts of Fundamentals for PSWs; and Additional and on-going relevant palliative education
