  • Bachelor’s degree or equivalent experience.
  • Minimum of 1 years of experience in a customer service or sales role, preferably within a healthcare or relevant industry setting.
  • Strong organizational skills with a keen eye for detail and accuracy and ability to manage multiple tasks and priorities in a dynamic environment
  • Excellent verbal and written communication skills.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
  • Have 1 year of experience working with a frail or elderly population
  • Be medically cleared for communicable diseases before engaging in direct participant contact, including a TB test.
  • Have all immunizations up to date before engaging in direct participant contact.
  • Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law.
Responsibilities
  • Act as the primary face of Habitat Health for and develop meaningful relationships with prospective participants, providing information, guidance, and support throughout the process.
  • Contribute to a program experience that Participants want to join, a team culture that cares and creates joy, and an environment where all participants and team members belong.
  • Continue to raise the bar. Constructively seek and share feedback and help us implement changes in order to improve clinical outcomes and experience for participants.
  • Exhibit and honor Habitat’s Values.
  • Contribute to center growth objectives by establishing strong relationships with referrals, and by championing PACE and building awareness in the community
  • Drive the enrollment process as the case manager for prospective participants, including reviewing, processing, and updating applications and required documentation, distributing relevant enrollment materials, and managing communications
  • Conduct tele, home visits and in-center evaluations for the prospective participant for potential eligibility for care.
  • Deliver high-quality customer service by addressing and resolving enrollment-related inquiries and concerns in a timely and professional manner.
  • Work collaboratively with Outreach Team, Enrollment RN, IDT, and center leaders to create a joyful and smooth enrollment process for prospective participants.
  • Monitor and report on enrollment metrics and trends to assist in strategic planning and decision-making. Prepare reports as needed for internal stakeholders.
  • Stay informed about and ensure adherence to relevant policies, regulations, and procedures related to the enrollment process.
Desired Qualifications
  • Ability to adapt to changes and learn new systems quickly.
  • Bilingual: Spanish strongly preferred.
  • Experience with enrollment software or CRM systems.
  • Experience working with the Medicare and Medicaid populations and within a healthcare organization.
  • Degree in healthcare, business administration, or marketing.

Habitat Health holds ribbon cutting for South Los Angeles PACE Center. February 27, 2026 Habitat Health, in collaboration with Kaiser Permanente, officially celebrated the opening of its South Los Angeles Program of All-Inclusive Care for the Elderly (PACE) Center today with a ribbon-cutting ceremony attended by community leaders and healthcare partners. The event marked a significant milestone in expanding access to coordinated, whole-person care for older adults age 55 and over in one of Los Angeles County's most medically underserved regions. "Today is more than the opening of a building. It marks a new chapter in how we care for older adults in South Los Angeles," said Sophia Guel-Valenzuela, Executive Director of Habitat Health South Los Angeles. "For too long, many older adults and their caregivers in this community have faced barriers to coordinated outpatient care. This center was built with those realities in mind. We are honored to be another PACE access point providing comprehensive, culturally responsive care directly for the neighborhoods that need it most." South Los Angeles experiences disproportionately high rates of chronic conditions such as diabetes, asthma, heart disease, and hypertension. Adults in the region face significantly higher rates of preventable hospitalizations and emergency department visits compared to other parts of the county. In fact, diabetes-related hospitalizations in South Los Angeles are 2.5 times higher than in West Los Angeles, highlighting serious gaps in preventive and coordinated care. "Opening the doors to this Habitat Health PACE center means opening doors to longevity, connection, and health for the community of South Los Angeles. Habitat Health's patients will have access to Kaiser Permanente's model, which is grounded in providing the right care, at the right place, at the right time - whether that's inside of our clinic walls or beyond. We're proud to play a key role in keeping this community healthy, independent, and active," said Dr. Bechara Choucair, chief health officer, executive vice president, Kaiser Permanente, and a Habitat Health board member. Habitat Health is both a PACE health plan and a direct care provider, offering integrated medical and social services under one roof. Services include primary and specialty care coordination, social services, day center activities, transportation, physical therapy, prescription management, nutrition counseling, home care coordination, dental services, and access to diagnostic and specialty services. Through its collaboration with Kaiser Permanente, participants also receive coordinated specialty and hospital-based care through Kaiser Permanente's regional network. For individuals who qualify for both Medicare and Medi-Cal, PACE services are typically available at no cost. The Compton center is now accepting referrals and applications.
