Salary Range: 110000 to 150000 (Currency: USD) (Pay period: per-year-salary)

Responsibilities:

• Perform comprehensive assessments of patients virtually and/or in person to gather medical histories and evaluate current health conditions.
• Create individualized care plans that specify health monitoring parameters and outline achievable health goals for each patient.
• Maintain accurate and timely documentation of patient interactions, assessments, education, and care plans within the EHR system. Ensure all records are compliant with policies and regulatory standards. 
• Assist in training other providers on remote monitoring workflow and best practices.
• Collaborate with Skilled Nursing physicians, Circadia physicians, nurses, and other healthcare professionals to provide a comprehensive approach to patient care.
• Participate in multidisciplinary team meetings to discuss and coordinate patient care strategies.

Qualifications:

• Medicare participation and ability to have company bill for services on your behalf
• Certification from ANCC (or equivalent) as an Adult, Family, Geriatric and/or Acute Nurse Practitioner
• 3+ years of Nurse Practitioner Experience working with complex adult populations
• 3+ years of telehealth experience
• Ability to practice independently with little or no supervision 
• Comfort using technology such as Google Suite, multiple EMRs, Slack 
• Comfortable in a dynamic and always evolving start up environment
• English required, Spanish preferred
