Requirements
  • Doctor of Medicine or Doctor of Osteopathic Medicine from an accredited institution.
  • Board certification or eligibility in Internal Medicine or Family Medicine.
  • An active, unrestricted Ohio medical license or the ability to obtain one.
  • Strong clinical skills in diagnosing and managing complex, age-related conditions.
  • Familiarity with interdisciplinary care models and comfort working with nurse practitioners, physician assistants, and other care-team members.
  • Knowledge of geriatric syndromes, mobility and fall-risk assessment, and cognitive evaluation tools.
  • The candidate must not be excluded from participation in Medicare or other Federal Health Care Programs.
  • A valid driver's license and ability to travel locally to patients and facilities in the Youngstown, Ohio area.
  • Strong verbal and written communication skills, empathy, and teamwork abilities.
Responsibilities
  • Provide direct medical care to older adults across clinic, long-term care, assisted living, and home settings.
  • Conduct comprehensive geriatric assessments covering medical, functional, cognitive, and psychosocial factors.
  • Diagnose and manage complex chronic conditions common in older adults, including dementia, frailty, cardiovascular disease, osteoporosis, and diabetes.
  • Monitor and adjust medications with attention to polypharmacy and age-related pharmacologic considerations.
  • Develop individualized treatment plans emphasizing prevention, health maintenance, and patient and caregiver education.
  • Guide patients and families through end-of-life care planning, advance directives, and palliative care coordination when appropriate.
  • Coordinate care with nurses, social workers, therapists, pharmacists, specialists, patients, families, and caregivers.
  • Collaborate with and support nurse practitioners and physician assistants caring for the shared patient panel.
  • Support the Regional Chief Medical Officer in developing and reinforcing clinical protocols and geriatric best practices.
  • Contribute geriatric clinical expertise to case reviews and provider training as needed.
  • Participate in quality improvement initiatives and clinical research as directed by the Regional Chief Medical Officer.
  • Maintain timely and accurate clinical documentation in compliance with regulatory, ethical, and organizational standards.
  • Maintain continuing medical education relevant to geriatric and home-based primary care.
Desired Qualifications
  • Fellowship training in Geriatric Medicine.
  • Experience in long-term care, skilled nursing facilities, home health, or palliative care.
