- An active Registered Nurse or Licensed Practical Nurse license in the state of Missouri is required.
- Home care experience is preferred but not required.
- Strong organizational and self-management skills are required.
- Strong and compassionate customer service skills are required.
- A valid driver's license is required.
- Complete admission assessments and reassessments for the in-home caregiver population.
- Provide nurse visits for adult and senior populations, including taking vital signs, medication management, monthly diabetic nail care, and laboratory work.
- Provide supervisory visits for caregiver services as needed.
- Provide instruction and education on medication.
- Home care experience.
Phoenix Home Care & Hospice provides in-home health services including skilled nursing, hospice and palliative care, private duty care, and support for developmental disabilities. Care is delivered at patients' homes and financed by Medicare, Medicaid, and private pay, which can reduce hospitalizations and delay move to facility-based living.
