CareCentrix

CareCentrix

Coordinates home-based post-acute care & DME

Overview

CareCentrix coordinates home-based healthcare to manage post-acute care and DME through a nationwide, fully credentialed network of nurses, social workers, and technology. It partners with health plans to guide patients to the right care at home, defining the best clinical path and orchestrating services so care happens where the patient is. This involves a holistic view of each patient’s needs, integrating clinical support, social work, and technology to track progress and coordinate resources. Compared with others, CareCentrix blends hands-on care with digital tools and a national network to offer end-to-end home-based care, focusing on post-acute and palliative care while streamlining DME management and reducing avoidable hospital readmissions. The goal is to improve patient outcomes and lower overall healthcare costs by delivering safe, effective care at home rather than in higher-cost settings.

BlueCross launches new medical equipment partnership.

BlueCross launches new medical equipment partnership. Published: 26 Mar 2026 BlueCross has collaborated with a new partnership with CareCentrix for durable medical equipment, including items like dressings, incontinence supplies, ventilators, and wheelchairs; materials that support treatment plans prescribed by physicians. The main focus is to relieve the financial burden on members due to the price increase of items like motorised leg and arm braces. This change aims to streamline DME services and reduce costs while ensuring statewide coverage and access to necessary equipment. According to Towards Healthcare, the U.S. durable medical equipment Market is projected to experience significant growth, with estimates suggesting the market size will increase from USD 74.62 billion in 2026 to approximately USD 121.34 billion by 2035, representing a compound annual growth rate (CAGR) of 5.55% from 2026 to 2035. This partnership is designed to provide the type and quality of care while improving affordability. CareCentrix coordinates with in-home delivery and setup through a specialised supplier. New network, same coverage, lower prices. Directing members through the CareCentrix network is expected to yield average savings of 22% on equipment, reducing costs for employer groups and members. Over 80% of DME items can ship directly to members' homes, coordinated by CareCentrix for sourcing and delivery. For complex equipment such as oxygen tanks, ventilators, hospital beds, or wheelchairs that require setup, CareCentrix will arrange in-home delivery and also look for expertise in a statewide network of specialised suppliers. If a member's current supplier is not present in the CareCentrix network, a new in-network provider will be assigned automatically. CareCentrix will also work directly with physicians to get any necessary orders or prescriptions to maintain an uninterrupted service streamlining. DME rental fees will cover the cost of maintenance, repairs, replacements, adjustments, supplies, and accessories. Rental fees also include equipment delivery services and set-up, education and training for member and their families, and nursing visits. Replacement of the rental equipment may occur when the rented item is irreparably damaged or if replacement is required during repair and/or maintenance of a specific item. Replacement or repair of an item that has been misused or abused by the member or the member's caregiver will be the responsibility of the member. A recent report by Towards Healthcare highlights that the U.S. durable medical equipment market is aligning with BlueCross's Border mission to improve health care quality, access, and affordability through innovative programs and partnerships.
