CenterWell Senior Primary Care(R) and Conviva Senior Primary Care(R) have partnered with Thyme Care to deliver comprehensive cancer care support for eligible patients across seven states Florida, Georgia, Nevada, North Carolina, South Carolina, Tennessee, and Texas strengthening care coordination and providing 24/7 access to oncology-trained clinicians through virtual navigation and proactive support that helps patients manage their diagnosis, stay connected to care teams, and avoid unnecessary hospital visits. Cancer care makes up over 10% of Medicare spending. Navigating treatment requires teamwork between primary and oncology care. Thyme Care partners with CenterWell physicians to offer ongoing, personalized support. This aims to improve coordination and communication while lowering costs. They offer close support from oncology-trained experts. This includes nurse practitioners, nurses, and social workers. These experts help with managing symptoms, scheduling appointments, and getting quick access to quality cancer care. They also connect patients to community resources that help with transportation and meals. They also provide palliative and urgent care support. Their focus is on managing symptoms early to prevent emergency visits.
