Overview

Circare is a non-profit that provides integrated health services and community-based support in Central New York. It serves as a designated Lead Health Home, coordinating a network of providers to help Medicaid beneficiaries manage serious mental illness, HIV/AIDS, and multiple chronic conditions. Services include comprehensive care management, health promotion, transitional care, and referrals to housing, vocational assistance, and other social supports, plus an ACT team with 24-hour support. It is funded by managed care contracts and government sources and aims to reduce preventable hospitalizations by coordinating care across providers to improve health outcomes.
