The programme provides ongoing support through an interdisciplinary care team including care managers, nutrition specialists, behavioural health professionals and medication support resources. It targets patients with conditions such as inflammatory bowel disease, liver disease and GERD, focusing on nutritional health, functional health, mental health and medication management. The initiative aims to bridge gaps in care when patients manage conditions independently, enabling earlier identification of issues and timely interventions. Cosan's care team works closely with Gastroenterology of the Rockies' medical providers to monitor patient progress and reinforce treatment plans.
