Timm brings over 15 years of healthcare leadership experience in population health, risk-based operations and provider networks. Most recently, she served as Senior Vice President of Operations at Optum, overseeing value-based medical group operations in Los Angeles South, supporting nearly 190,000 patients. At Evergreen, Timm will oversee operations and lead execution across the organisation as it continues to expand access to value-based kidney care. The collaboration combines Evergreen's nephrology expertise with Phamily's AI capabilities to improve care for patients with chronic kidney disease. Evergreen Connected Care enables nephrology practices to deliver population-wide care through AI-enhanced clinical decision support, comprehensive care planning, and omni-channel patient communication. The service aims to improve patient engagement whilst reducing administrative burden on nephrologists, allowing them to spend more time with patients.
