RPM Healthcare, a Newark-based remote patient monitoring company, has launched an enhanced platform and Care Coach model to address nurse burnout in physician practices. The system offloads chronic disease monitoring and patient outreach to licensed nurse Care Coaches, reducing workload on practice teams. According to a 2025 survey of over 2,600 nurses, 65% report high stress and 40% are uncertain about remaining in the profession. RPM Healthcare's Care Coaches manage routine chronic care responsibilities, with 98% of patient alerts resolved at their level before reaching clinical staff. The upgraded platform includes dynamic prioritisation of high-risk patients, embedded appointment scheduling and streamlined escalation workflows. The company has raised $295 million across three funding rounds to support its chronic care coordination model, which integrates with existing clinical workflows.
