Overview

Utah Partners for Health provides primary and preventive care to uninsured and underserved residents in Salt Lake and Tooele counties as a Federally Qualified Health Center. It operates with a patient-centered medical home model and offers integrated services including medical, dental, behavioral health, pharmacy, vision, and substance-use treatment, plus a mobile clinic and prenatal clinic to reach hard-to-reach populations. Funding comes from federal and government sources, Medicare/Medicaid, private insurance, and sliding-scale patient payments, with support from partnerships that donate in-kind services. Its goal is to expand access to affordable, comprehensive health care for underserved communities as needs grow, including medication-assisted treatment for opioid addiction and various counseling options.
