Requirements
  • A New York State nursing license is required.
  • At least two years of licensed practical nurse experience are required.
  • At least one year of experience in community healthcare services is required.
  • The role requires strong written and verbal communication skills.
  • Knowledge of Microsoft Office tools is required.
  • The role requires the ability to provide crisis intervention using interpersonal skills and a high level of maturity and responsibility.
  • Specialized training in intellectual and developmental disabilities is preferred.
  • Basic knowledge of human services is preferred.
  • Bilingual Spanish and English speaking ability is preferred.
Responsibilities
  • Coordinate medical and psychiatric care and medications for residents, ensuring proper documentation, follow-up, and regulatory compliance.
  • Assist with hands-on training of residential staff to maintain ongoing compliance and current practices.
  • Handle telephone calls and inquiries, provide necessary follow-up, and notify families and management when needed.
  • Act as liaison between the residence, day program, physicians, and families regarding resident medical issues.
  • Inform and instruct managers, coordinators, and residence staff about treatment and medication changes and verify that instructions are carried out on the next shift.
  • Update the registered nurse daily on each supported person's medical status.
  • Review prior-day medication documentation and notify the registered nurse of detected errors.
  • Review prior-day vital signs, glucose readings, and bowel movement charts and notify the registered nurse of irregularities.
  • Communicate pertinent medical information to the registered nurse, manager, day programs, and families as needed.
  • Write communication log notes concerning medical visits or medical situations and update medical summaries.
  • Notify the registered nurse and manager of medication or treatment changes and plan follow-up.
  • Report residents' medical symptoms to the registered nurse and follow the registered nurse's instructions.
  • Notify the registered nurse, manager, and behavior specialist of psychotropic medication changes and obtain necessary consents in conjunction with the registered nurse.
  • Implement directives issued by the registered nurse and manager as needed.
  • Coordinate medication administration, including verifying prescriptions, ordering medications, completing regulatory documentation, and overseeing staff medication administration in conjunction with and under the supervision of the registered nurse.
  • Maintain medication inventory, including prescription medications, over-the-counter medication, and required first-aid supplies, and routinely check expiration dates.
  • Transport and accompany residents to scheduled medical appointments and complete required follow-up after appointments.
  • Maintain medically related documentation, including medication administration records and physician consultations.
  • Complete documentation before the end of each shift and report issues and concerns to the registered nurse and residence manager.
  • Complete occurrence reports as required.
  • Complete residential habilitation notes, progress notes, and other required documentation.
  • Review and make daily entries in the house log book regarding pertinent resident and residence issues.
  • Monitor resident progress and notify appropriate parties of concerns, behavioral changes, or progress.
  • Apply emergency procedures during crises, including fire-safety procedures.
  • Provide residents with instruction or assistance in daily living, rehabilitation, or social skills according to the Individual Service Plan.
  • Supervise residents' medication intake as prescribed under the approved Medication Administration Procedure.
  • Administer medications as needed.
  • Review Precision Care at the beginning of each shift and follow up on medical, pharmacy, and nursing entries.
  • Enter new medications or medication changes on medication administration records, complete resident-specific information, and retain current prescriptions.
  • Provide ongoing training to residents and staff in areas identified in the Individual Service Plan in conjunction with the registered nurse and manager.
  • Provide client-specific hands-on training to staff members as directed by the registered nurse.
  • Attend meetings and in-services and participate in staff meetings and mandatory annual in-services as directed by the residence manager and registered nurse.
