Requirements
  • Graduation from an accredited School of Professional Nursing as a Licensed Practical Nurse.
  • Current and valid New Jersey Professional Nursing License.
  • Current Basic Life Support certification.
  • Ambulatory Surgery Technician Certification is required for employees who sterilize surgical and medical instruments; those employees must pass the Certification Board for Sterile Processing and Distribution Ambulatory Surgery Technician Exam within six months of hire.
  • Pass Spanish proficiency testing within 30 days of notification when bilingual.
  • Ability to work with computers and software applications suitable for a health care environment.
  • Computer literacy and proficiency.
  • Ability to maintain client confidentiality.
  • Ability to kneel, bend, and perform light lifting.
  • Ability to write and speak clearly using the English language and hear at normal speaking levels in person and by telephone.
Responsibilities
  • Provide professional nursing services on a regular or emergency basis during business hours and as assigned under the supervision of the Nurse Care Manager and/or Director of Nursing.
  • Communicate with physicians and team members about changes in patients' clinical conditions, including diagnostic study results and symptoms.
  • Prepare assigned patients for examinations and treatments following established nursing procedures.
  • Assist providers during examinations and treatments and provide clinical services directed by written orders within the Licensed Practical Nurse scope of practice.
  • Observe patients, record significant conditions and reactions in medical records, and notify managers or providers of patient conditions, drug or treatment reactions, and significant incidents.
  • Take temperature, pulse, blood pressure, and other vital signs; report abnormal findings to the provider or Registered Nurse.
  • Complete daily chart checks and prepare patient charts before and during patient visits.
  • Assign tasks to ancillary staff in the absence of the Nurse Care Manager under nursing administration and/or provider supervision.
  • Instruct and counsel patients and authorized significant others regarding current health conditions and health maintenance.
  • Assist with patient referrals and follow-up.
  • Maintain accurate and complete nursing documentation and confidentiality of patient information.
  • Assist providers and clinical team staff with ancillary testing and other clinical duties.
  • Receive callers, determine their reasons for visiting the office, provide required information, and address requests within the Licensed Practical Nurse scope of practice.
  • Apply accepted nursing standards in decision-making and patient care.
  • Prepare and maintain patient rooms, sterile instruments, equipment, and supplies.
  • Maintain and use health-record indexes, storage, recall, and retrieval systems, including automated data-processing systems.
  • Enter data into computer programs, including the electronic medical-record system; scan laboratory results and reports into patient charts.
  • Complete patient referral documents and pre-certifications within required time frames, follow referral guidelines, provide clinical information to specialists, and maintain assigned logbooks.
  • Maintain essential medical records and files.
  • Set self-management goals and assist the clinical team with goals.
  • Notify patients about documents required for registration and verify insurance eligibility when scheduling appointments.
  • Follow scheduling rules, make appointment confirmation calls, and follow up on no-shows, including DNKA.
  • Participate in population-management activities using preventive and health-maintenance templates, CEM, Business Objects reports, UDS reports, and performance, quality-improvement, and quality-assurance projects.
  • Rotate on an urgent basis and assist various departments and sites as directed to meet staffing needs.
  • Participate in staff education, in-service programs, departmental projects, form completion, reports, quality-assessment activities, performance improvement, and continuous quality-improvement activities.
  • Seek guidance and validation from appropriate clinical or management resources when necessary.
  • Greet patients, clients, and customers by telephone or in person and promote positive patient relations.
  • Participate in workflows within the Licensed Practical Nurse scope of practice.
  • Assist staff with Spanish interpretation when bilingual in Spanish.
  • Attend mandated job-related training, annual reviews, department in-services, and staff meetings.
  • Maintain current licensure and complete continuing-education requirements as directed by the New Jersey State Board of Nursing.
  • Maintain regulatory requirements, patient confidentiality, and organizational policy compliance.
  • Perform other assigned duties and assume additional responsibilities as directed by the Nurse Care Manager.
Desired Qualifications
  • Two years of experience as a Licensed Practical Nurse is preferred; previous experience in a public health or primary care setting is beneficial.
  • Bilingual English/Spanish ability is preferred.
