Requirements
An Associate Degree in Human Service or equivalent is required; a combination of education and experience may be considered.
At least three years of experience working with vulnerable individuals is required.
A valid New York State driver's license is required.
A registered and insured vehicle is required.
New York State Child Abuse Registry clearance is required.
Mobility sufficient to drive to and participate in meetings at clients' homes or community facilities that may not meet federal accessibility standards is required.
Manual dexterity sufficient to work with children and adults in a challenging and active environment is required.
Responsibilities
Conduct comprehensive interviews with individuals experiencing personal and family adjustments, potential foster care situations, financial struggles, employment issues, and concerns related to food, clothing, housing, or physical or mental impairments.
Gather information about clients' medical, psychological, and social factors to assess the nature and severity of their problems.
Provide individual or group counseling services.
Assist clients in developing plans to address their needs and access internal strengths and external resources to improve social functioning.
Meet with clients as required by funding sources, monitor their progress, and provide guidance to modify attitudes and behaviors for positive change.
Connect clients with community resources and organizations to establish support networks.
Complete necessary reporting forms and maintain accurate client case files.
Communicate effectively with supervisors and funding sources.
Monitor client safety and promptly notify relevant parties of potential safety issues.

Responsibilities
The Case Worker plays a crucial role in providing support to clients facing a wide range of challenges. They conduct comprehensive interviews with individuals dealing with personal and family adjustments, potential foster care situations, financial struggles, employment issues, and various other concerns related to food, clothing, housing, and physical or mental impairments. Through these interviews, the Case Worker gathers important information about the clients' medical, psychological, and social factors, enabling them to assess the nature and severity of the problems. They then offer counseling services on an individual or group basis, assisting clients in developing plans to address their needs and empowering them to tap into their inner strengths and external resources for improved social functioning. The Case Worker regularly meets with clients as required by funding sources, monitoring their progress and providing guidance to modify attitudes and behaviors for positive change. Additionally, they connect clients with community resources and organizations, establishing a network of support. Administrative tasks such as completing necessary reporting forms, maintaining accurate client case files, and ensuring effective communication with supervisors and funding sources are also part of their responsibilities. The Case Worker remains vigilant about client safety, promptly notifying relevant parties of any potential safety issues.
This position is Full Time at 35 hours a week. Pay for this position is $22.00 per hour
