Duties and Responsibilities:

·Conducts home assessments (and virtual assessments, as needed) for prospective members and re-assessments of current members using the Uniform Assessment System (UAS); make enrollment and continued eligibility determinations by utilizing clinical expertise and critical thinking skills. As needed, conducts assessments in settings other than home as needed, e.g. skilled nursing facilities or virtually

·Navigates multiple technology/digital platforms such as UAS software, electronic medical records, TruCare, etc., to conduct and document comprehensive assessments accurately and efficiently

·Completes assessment of service needs per CMT and member's family/caregiver’s request to determine service needs or any necessary adjustments

·Reviews previous nursing assessments/medical notes to determine necessary adjustments/updates in Care Plan and/or to assist with nursing home admission

·Conducts in-person Care Management visits in support of person-centered service planning

·Liaisons between the member and the CMT; assesses home environment and psychosocial status

·Provides feedback to home care agencies and CMT regarding home care issues and contract nurses and aides performance

·Provides grass roots, community-based training for frail population care including self-care techniques and prevention strategies

·Ensures (prospective) members and significant others/responsible parties understand and are in agreement with enrollment in a managed long-term care plan

·Additional duties as assigned

Minimum Qualifications:

·New York State RN license

·Ability to travel around downstate New York which includes the 5 boroughs, Long Island, Rockland, and Westchester.

·Prior work experience requiring technological proficiency including the ability to navigate multiple technology platforms and modalities

·Prior work experience requiring intermediate Microsoft Word, Excel, and Outlook skills

·Prior work experience requiring time management, critical/creative thinking, communication, and problem-solving skills

·For PEDS positions only: 1 year of pediatric clinical field experience and/or experience with families and child serving systems, including child welfare and/or medically fragile/developmentally disabled populations

Preferred Qualifications:

·Work experience using electronic patient health information (PHI) database usage especially UAS

·Previous field-based experience assessing, planning, and evaluating member's care by making home or facility visits for intake/reassessments or start of care (SOC) Community Home Health Agency (CHHA) visits

·Work experience preferred in one or more of the following areas, geriatrics, home care, discharge planning, case management, and/or medical surgical nursing

·Knowledge of health insurance, Medicaid, Medicare and MLTCP

·Experience working with a frail adult or elderly population with the ability to determine appropriate care plans and services for frail population as well as negotiate initial service plans so that members and families are in agreement

·Language preferences - Spanish, Russian, French, Creole, Mandarin, Cantonese

·Demonstrated ability handling heavy caseloads

·NY state driver’s license

Greater New York City Area (NY, NJ, CT residents): $100,000 - $120,000
All Other Locations (within approved locations): $71,600 - $106,505

As a candidate for this position, your salary and related elements of compensation will be contingent upon your work experience, education, licenses and certifications, and any other factors Healthfirst deems pertinent to the hiring decision.

Volunteer performers at the Sept. 12 Healthfirst Mid-Autumn Festival celebration in Elmhurst with members of the Healthfirst leadership team. Healthfirst celebrated the new fall season with its Mid-Autumn Festival on Sept. 12 at the CC Moorehead Playground in Elmhurst. Festivities included cultural performances, health education and information on social services from community-based organizations. The celebration featured a Lion dance performance, a traditional Chinese dance believed to bring good fortune, and healthy living activities for all ages such as crafting and gardening. Health services provided at the event included Medicare guidance, informing older adults about coverage options, savings programs and benefits available to eligible older adults. A Chinese chive planting activity taught attendees about healthier cooking habits while connecting wellness education to cultural traditions. A "Show Me the Sugar" activity, sponsored by Healthfirst, helped attendees identify sugars in beverages and encouraged healthy hydration choices. Representatives from the Office of Queens Borough President Donovan Richards also spoke to attendees about local resources that support their everyday well-being. Healthfirst officials in attendance included Errol Pierre, chief growth officer, Stephanie Wu, vice president of state programs, and William Wang, assistant vice president of Asian market development. "Healthfirst is dedicated to ensuring our community members have the right health plan that fits their specific healthcare needs, while also recognizing their cultural traditions," Wang said. "The Mid-Autumn Celebration connects our neighbors in Queens with health resources while providing a venue where community members can connect and celebrate. We wish our members and their neighbors a wonderful holiday filled with good health and fortune." A spokesperson for Northwell Health said the event supported its goal of breaking down barriers to healthcare by bringing its care services and wellness resources directly to the community. The healthcare provider emphasized that by providing this support, it helps ensure every family has the support and access it needs to live healthier lives. "Northwell Health is deeply honored to collaborate with the Elmhurst community for this year's Mid-Autumn Festival," the spokesperson said. "This celebration is not only a wonderful time to honor rich cultural traditions with families and neighbors, but it is also a vital opportunity for us to connect directly with residents." Alina Moran, CEO of NYC Health and Hospitals/Elmhurst, said that the hospital is committed to promoting and protecting the holistic health of community members to its fullest extent. Nonprofit recognizes Queens pharmacies for improving health of members with Medicare. Three pharmacies in Queens were recognized by Healthfirst, a nonprofit health insurer serving more than two million New Yorkers, for their work to improve the health of members covered under the nonprofit's Medicare plan. Winners of the nonprofit's inaugural Pharmacy Excellence Awards, which were presented to 12 community pharmacies across the city, were announced by Healthfirst Oct. 14. The Queens winners include All RX Pharmacy 1 and Arcadia Health Pharmacy, both located in Flushing, and Victory Pharmacy in Jackson Heights. Recipients of the awards, who were among hundreds of participating community pharmacies, were honored for achieving the city's highest rates of medication adherence and proactive pharmacy interventions to close gaps in care in 2024. The awards align with the mission of the Healthfirst program, which is aimed at equipping community and independent pharmacists with new tools and technology to help close care gaps, improve medication adherence and provide timely support for members on Medicare. According to the nonprofit, the winning pharmacies across Queens, the Bronx, Brooklyn and Manhattan are trusted healthcare partners in their neighborhoods. "Community pharmacies know their patients by name and understand the challenges they face every day, and that they want more time to educate patients with medication management but also have the demands of running a small business," said Bhavesh Modi, vice president of pharmacy at Healthfirst. "Through this program, we're giving pharmacists the tools and resources they need to make an even greater impact on the health of their communities." Medication adherence is one of the most important factors in improving long-term health outcomes for Medicare members, according to the nonprofit. Pharmacies enrolled in the program have helped significantly boost adherence rates, ensuring more Medicare members are taking their medications as prescribed. "It can be daunting to manage several medications without support, especially for our seniors," said Bronx state Sen. Gustavo Rivera, who chairs the Senate Health Committee. "I extend my congratulations to the local pharmacies being honored and commend Healthfirst for recognizing the role that independent pharmacies play in helping New Yorkers lead healthier lives."
