Requirements
- A nurse practitioner must graduate from a Commission on Collegiate Nursing Education-accredited nursing program with a Master's Degree in Nursing.
- A nurse practitioner must hold national certification from a recognized Advanced Practice Registered Nurse Certification Center.
- A nurse practitioner must be licensed or eligible for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
- A physician assistant must hold a Bachelor's Degree as a Medical/Surgical Physician Assistant, as applicable.
- A physician assistant must graduate from an Accreditation Review Commission on Education for the Physician Assistant-approved Physician Assistant training program.
- A physician assistant must be certified by the National Commission on Certification of Physician Assistants.
- A physician assistant must be licensed or eligible for licensure by the West Virginia Board of Medicine.
- Basic Life Support certification is required; additional Advanced Life Support training may be required at the department's discretion.
- Candidates must demonstrate proficiency with computers.
- Candidates must demonstrate strong communication skills.
- Candidates must meet the physical demands of heavy or hard work, including lifting, moving, stooping, reaching, positioning patients, standing, walking, and carrying patients, materials, and equipment weighing 40 or more pounds.
Responsibilities
- Manage comprehensive and individualized care for family medicine patients and their families in the ambulatory setting.
- Complete consults, pre-procedure evaluations, acute visits, and wellness visits.
- Complete integrated assessments.
- Order additional tests or consults in collaboration with family medicine faculty or according to established clinical guidelines and protocols.
- Perform diagnostic tests and communicate results according to established clinical guidelines and scope of practice.
- Consult and communicate with clinic and hospital staff regarding abnormal patient findings.
- Perform comprehensive clinical evaluations and document findings in the medical record.
- Coordinate appropriate services and tests for outpatient follow-up appointments.
- Develop individualized plans of care that include teaching, follow-up planning, and referrals to other healthcare team members.
- Provide care for family medicine patients across the age continuum within the scope of education and certification.
- Identify unstable or complex patient-care situations and facilitate appropriate intervention in the clinic or hospital setting.
- Recognize emergency situations and initiate effective emergency care.
- Coordinate follow-up procedures and tests for family medicine patients in the ambulatory setting.
- Assist with collecting outcome measures.
- Apply critical thinking and diagnostic reasoning to identify, evaluate, and manage individual patient and family needs.
- Use equipment safely.
- Base decisions and actions on scientific principles, established standards of care, and research findings.
- Provide education to nursing staff, hospital staff, students, patients, families, and the community using appropriate teaching strategies.
- Promote an environment that supports patient and family learning preferences, priorities, and readiness to learn.
- Maintain an updated knowledge base throughout the year.
- Complete all identified clinical competencies.
- Serve as a resource for staff caring for family medicine patients.
- Provide educational programs to professional and paraprofessional personnel within the area of expertise.
- Participate in community programs and distribute relevant literature.
- Document each patient's plan of care, interventions, and evaluations clearly and concisely in the medical record according to approved policies and compliance standards.
- Collaborate with leaders and staff to achieve improvements in patient outcomes and care.
- Lead the development and implementation of changes in family medicine practice that positively affect patient outcomes.
- Work collaboratively with medical and nursing staff to facilitate problem-solving and provide professional support.
- Participate in and initiate interdisciplinary approaches to patient care, program development, and education, serving as a professional role model.
- Promote innovation in the expanded role and delivery of care to family medicine patients and families in inpatient and ambulatory settings.
- Participate in and provide leadership for committees and task forces.
- Document hours and submit billing reports as appropriate.
- Review and interpret family medicine data, including patient registries, clinical and financial reports, length of stay, access to care, and cost-per-case reports, and recommend or implement changes as necessary.
- Participate in clinical trials and research studies with the Principal Investigator.
- Assist family medicine faculty, administrators, and directors with collecting outcome measures.
- Perform other assigned duties and projects.
Desired Qualifications
- Family medicine experience is preferred.
- Acute Care Nurse Practitioners and Family Nurse Practitioners are considered.
