- Hold a current Licensed Practical Nurse license in Virginia.
- Have a minimum of one year of nursing experience.
- Hold current CPR certification.
- Demonstrate communication, organizational, and human relations skills.
- Function with minimal supervision.
- Manage multiple projects and changing priorities.
- Employ critical thinking skills.
- Accept assignments of clients from the Registered Nurse Case Manager.
- Provide nursing care through utilization of home assessment, nursing diagnoses, and physician-approved care plans.
- Perform treatments ordered by the physician and delegated by the Registered Nurse Case Manager.
- Observe signs and symptoms and report them to the Registered Nurse Case Manager.
- Note changes in the physical or emotional status of clients.
- Promote client and caregiver independence in self-care.
- Utilize electronic documentation of client care and comply with agency policies regarding timely submission of documentation and data synchronization.
- Follow agency policies regarding universal precautions and safety guidelines.
- Fulfill other assignments as requested by the Registered Nurse Case Manager, Vice President of Clinical Services, or Director of Professional Services.
