Requirements
  • Current, unencumbered Registered Nurse license in the state of employment.
  • At least 1 year of Registered Nurse experience in hospice, home health, acute care, long-term care, oncology, or another high-acuity setting.
  • Current cardiopulmonary resuscitation certification.
  • Valid driver’s license, auto insurance, and reliable transportation.
Responsibilities
  • Deliver nursing care to hospice patients.
  • Manage and implement individualized plans of care.
  • Support and lead interdisciplinary team members, including Licensed Practical Nurses, Certified Nursing Assistants, and Home Health Aides.
  • Assess and address patients’ physical, emotional, spiritual, and environmental needs.
  • Document all care provided and ensure compliance with hospice protocols.
  • Educate families and caregivers on care instructions, medications, and safety.
  • Participate in the on-call rotation and provide after-hours care as needed.
  • Collaborate with physicians, referral sources, and the care team.
  • Assist with referrals, regulatory documentation, and payer compliance.
  • Travel to various Gentiva hospice locations as assigned to fill urgent needs.
