Requirements
  • The candidate must be a Registered Nurse with a current license in the state of employment.
  • The candidate must have at least one year of experience as a Registered Nurse in an acute care setting.
  • The candidate must have a valid driver's license and auto insurance in their name as a driver.
  • The candidate must be capable of meeting all physical demands of the role.
  • The candidate must be able to administer medication as prescribed and perform duties within the scope of practice established by the state.
  • The candidate must complete clinical documentation according to agency protocol and Medicare and federal guidelines.
  • The candidate must follow agency policies, procedures, rules, and regulations.
Responsibilities
  • Provide one-on-one, home-based nursing care under the direction of an attending physician.
  • Coordinate care as part of an interdisciplinary team and provide clinical care under physician direction.
  • Assume accountability for assessing patient conditions and developing and managing plans of care.
  • Perform OASIS assessments to develop individualized care plans and adjust them as patient needs change.
  • Administer medication as prescribed by the physician.
  • Prioritize visits for high-risk patients to help decrease rehospitalizations.
  • Oversee and supervise the total care of patients provided by nurse aides and licensed practical nurses.
  • Manage multidisciplinary care and promote continuity through appropriate admissions, transfers, and discharges.
  • Educate patients and family members about disease processes using teach-back methods.
  • Advocate for patients and ensure patient safety.
  • Collaborate with the interdisciplinary team and physicians to ensure optimal care.
  • Communicate schedule or availability changes to schedulers or supervisors.
  • Attend in-service training and mandatory agency meetings.
