Requirements
- Graduate of a Commission on Collegiate Nursing Education accredited nursing program with a master's degree in nursing, for nurse practitioner candidates.
- National certification by one of the recognized Advanced Practice Registered Nurse certification centers, for nurse practitioner candidates.
- Licensure or eligibility for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in Pennsylvania, for nurse practitioner candidates.
- American Heart Association Basic Life Support certification is required, with additional Advanced Life Support training at the discretion of the department, for nurse practitioner candidates.
- A bachelor's or master's degree as a Medical/Surgical Physician Assistant is required, for physician assistant candidates.
- Graduate of an Accreditation Review Commission on Education for the Physician Assistant approved physician assistant training program, for physician assistant candidates.
- Certification by the National Commission on Certification of Physician Assistants is required, for physician assistant candidates.
- Licensure or eligibility for licensure by the Pennsylvania Board of Medicine is required, for physician assistant candidates.
- American Heart Association Basic Life Support certification is required, with additional Advanced Life Support training at the discretion of the department, for physician assistant candidates.
- Proficiency with computers is required.
- The role requires strength and/or stamina for lifting, moving, stooping, reaching, positioning patients, standing, walking, and carrying patients, materials, and equipment weighing 40 or more pounds.
- The role requires working closely with others, protracted or irregular hours, biohazards, infectious diseases, the deceased, hands in water, and electrical hazards associated with patient-care equipment.
Responsibilities
- Manage comprehensive and individualized care for primary care patients and their families in the ambulatory setting.
- Conduct consultations, pre-procedure evaluations, acute visits, and wellness visits.
- Complete integrated assessments.
- Order additional tests or consultations in collaboration with primary care faculty or according to established clinical guidelines and protocols.
- Perform diagnostic tests and communicate results according to established clinical guidelines and scope of practice.
- Consult and communicate with clinic and hospital staff regarding abnormal findings in patient evaluations.
- Perform comprehensive clinical evaluations and document findings in the medical record.
- Facilitate patient follow-up by coordinating appropriate services and tests for clinic follow-up appointments, as applicable.
- Develop comprehensive individualized plans of care for primary care patients, including teaching, follow-up planning, and referrals to other healthcare-team members.
- Provide care for patients across the age continuum appropriate to the individual's education and certification.
- Identify unstable or complex primary care situations and facilitate appropriate intervention in the clinic or hospital setting.
- Recognize emergency situations and initiate effective emergency care.
- Evaluate primary care patients and coordinate follow-up procedures and tests in the ambulatory setting, as applicable.
- Assist with collecting outcome measures, as applicable.
- Use critical thinking and diagnostic reasoning in clinical decision-making by identifying, evaluating, and managing the individual needs of patients and families.
- Use equipment safely.
- Base decisions and actions on relevant scientific principles, established standards of care, and research findings.
- Use teaching and learning theory and appropriate teaching strategies to educate nursing staff, hospital staff, students, patients, families, and the community.
- Promote an environment that facilitates learning by responding to patient and internal preferences, priorities, and readiness to learn.
- Update professional knowledge throughout the year.
- Complete all identified clinical competencies.
- Serve as a resource person for staff caring for primary care patients.
- Provide educational programs to professional and paraprofessional personnel in the area of expertise.
- Participate in community programs and distribute literature as appropriate to the area of expertise.
- Document the plan of care, interventions, and evaluations clearly and concisely for every patient visit in the medical record according to approved hospital and University Health Associates policies and compliance standards.
- Collaborate with leaders and staff to achieve outcome-based improvements in patient care.
- Provide leadership in developing and implementing changes in primary care practice that positively affect patient outcomes.
- Work collaboratively with medical and nursing staff to facilitate problem-solving and provide professional support.
- Participate in and initiate interdisciplinary approaches to patient care, program development, and education, and serve as a professional role model.
- Promote and practice innovation in the expanded role in delivering care to primary care patients and families in inpatient and ambulatory settings.
- Participate in and provide leadership for committees and task forces.
- Document hours and submit billing reports, as appropriate.
- Review and interpret primary care data, including patient registries, clinical and financial reports, length of stay, access to care, and cost-per-case reports, and recommend or implement changes as necessary.
- Participate in clinical trials and research studies with the Principal Investigator.
- Assist primary care faculty, administrators, and directors in collecting outcome measures.
- Perform other assigned duties and projects.
Desired Qualifications
- Primary care experience is preferred for nurse practitioner candidates.
- Eligible new graduates will be considered for nurse practitioner candidates.
- Acute Care Nurse Practitioners and Family Nurse Practitioners are considered.
- Primary care experience is preferred for physician assistant candidates.
- Eligible new graduates will be considered for physician assistant candidates.
- A master's degree as a Physician Assistant is preferred.
