An active and unrestricted California Physical Therapist license is required.
Graduate of an accredited Physical Therapy program.
Strong communication skills and the ability to work independently.
Reliable transportation is required for in-person visits.
Responsibilities
Provide physical therapy evaluations, treatments, and individualized care plans focused on mobility, strength, balance, pain management, and quality of life.
Monitor patient progress, maintain clinical documentation, and collaborate with facility staff and the healthcare team.
