Requirements
  • Be a member in good standing of the Quebec Order of Social Workers and Marriage and Family Therapists (OTSTCFQ).
  • Hold a university bachelor's or master's degree in social work.
  • Hold a valid driver's license to perform home visits in Baie-Comeau and surrounding areas.
Responsibilities
  • Assess clients' social functioning, support networks, and vulnerability directly in their living environments.
  • Intervene in complex clinical situations involving loss of autonomy, isolation, abuse, and crisis situations.
  • Develop and coordinate psychosocial intervention plans with clients, their relatives, and the interdisciplinary team, including nurses, occupational therapists, and physicians.
  • Lead or collaborate on legal protection processes, including homologation of protection mandates and the opening of protective regimes or tutorships.
  • Coordinate referrals and connections to partner resources in the health network and local community.
