  • Ability to perform the physical and mental tasks associated with the position, including sitting, standing, walking, bending, stooping, reaching, and lifting or carrying up to 50 pounds.
Responsibilities
  • Provide and oversee appropriate and timely resources, interventions, and services for selected hospital patients.
  • Collaborate with hospital-based case management and clinical staff, the patient's primary provider, the High Risk Registered Nurse Care Coordinator, practice team members, and community partners on care plans for high-risk patients.
  • Assess patients' social determinants of health, including living situation, safety, abuse and neglect, risk to self or others, substance misuse, financial resources, behavioral health history, barriers to care, cultural issues, coping style, life transitions, understanding of illness or disease processes, primary care provider involvement, and advance directives.
  • Provide psychosocial evaluations and assessments of patients, families, and caregivers.
  • Assess learning and coping styles, cultural issues, family resources, and readiness to participate.
  • Recommend direct services and referrals to community resources based on patient and family needs.
  • Advocate for patients and families to ensure access to services.
  • Review readmissions and implement interventions to prevent further readmissions.
  • Provide direct interventions to patients and families based on assessment, need, and patient consent.
  • Provide financial information to patients and families.
  • Facilitate Medicaid applications and other community financial assistance as needed.
  • Act on social work referrals within one business day.
  • Work collaboratively with the interdisciplinary team and community partners.
  • Assist with seamless transitions across the continuum of care.
  • Facilitate community support groups according to identified needs.
  • Maintain effective working relationships with community partners.
  • Facilitate advance directives as requested.
  • Conduct patient and family meetings.
  • Help patients and families understand and accept medical recommendations and support their decisions.
  • Facilitate team conferences, discharge planning, and family conferences.
  • Meet patients at practices, the hospital, in the community, and in homes as needed.
  • Maintain timely, precise, and relevant documentation.
  • Organize workload and set priorities based on patients' medical needs, preferences, service availability, and patient or family compliance.
  • Maintain current knowledge of organizational policies and procedures.
  • Maintain clinical competence in hospital social work practice.
  • Participate in peer review with team members.
  • Provide clinical supervision to undergraduate or graduate social work students in fieldwork placements when assigned.
  • Develop and implement process improvement activities on assigned units.
  • Identify opportunities to improve hospital-wide systems for delivering social services.
  • Identify high-risk patients and families and refer them to Quality and Risk Management.
  • Stay current with DNV, Centers for Medicare & Medicaid Services, and state regulations pertaining to social services.
  • Assist with writing policies, memos, and patient and family education materials.
  • Assist in planning and developing programs and special projects.
  • Serve on assigned hospital committees.
  • Perform assignments requested by the Director of Care Coordination.
Desired Qualifications
  • Licensed Independent Clinical Social Worker licensure is preferred.
  • Knowledge of end-of-life issues is preferred.
  • Knowledge of community agencies and resources is preferred.
  • A minimum of two years of experience in a health care setting is preferred.
