Requirements
  • Graduate of a college level program in physical therapy, occupational therapy, or an accredited two-year program for a Physical Therapist Assistant or Occupational Therapist Assistant and minimum of 2 years’ experience in social service or health care field; or any combination of education and experience, which would provide an equivalent background.
  • Current active, valid and unrestricted license or certification as a physical therapist assistant or occupational therapist assistant in applicable state required.
  • Certification and/or licensure appropriate to field of specialty as required.
Responsibilities
  • Performs telephonic and/or virtual assessments to identify participants needs. Provides recommendations to MCO for type and hours of supportive services required.
  • Conduct objective assessments for program participation to determine the appropriate level of support and services required.
  • Obtain participant history to inform the comprehensive assessment. Complete assessments annually or more frequently as needed in accordance with applicable program requirements and participants needs.
  • Educate program participants and MCO representative on options for home modifications, DME, assistive technology, or other adaptive equipment. Assists clinical team with the recommendations for equipment and services as needed.
  • Collaborate with the support team to report observations and outcomes. Document all member encounters per documentation standards.
