A physician’s license to practice in Indiana, or the ability to obtain licensure, is required.
DEA registration is required.
An Indiana license to prescribe controlled substances (CSR) is required.
Proof of residency completion with board eligibility and board certification is required.
BLS is required.
Demonstrate effective interpersonal skills.
Be able to lift up to 10 pounds.
Be able to perform prolonged or extensive standing and walking.
Be able to perform considerable reaching, stooping, bending, kneeling, and crouching.

Job Summary:
Maple City Health Care Center (MCHCC) is a Federally Qualified Health Center dedicated to improving our community’s health by making quality comprehensive healthcare accessible to all. We strive for a healthy community where everyone is cared for.
The Family Physician is a member of a cross-disciplinary team that includes health care providers, medical assistants, nurses, behavioral health providers, and other support staff. The Family Physician supports the mission and values of Maple City Health Care Center (MCHCC) by providing primary care to patients of all ages and genders.
Duties and Responsibilities:
Provides primary care for MCHCC patients in clinic
Provides clinical leadership for the care team in which they are located
Serves as a collaborating physician on collaborative practice agreements for nurse practitioners and midwives
Utilizes EMR effectively in patient care
Utilizes multiple resources for MCHCC communication
