Requirements
  • A Bachelor's degree in Social Work, Human Services, Psychology, or a related field is required.
  • If the degree is in social work, current Minnesota licensure is required.
  • A Bachelor's degree in a non-Human Services field requires one year of related experience.
  • Proficiency in Microsoft, Google, and care-related platforms is required.
  • The ability to pass mandated background checks is required.
  • A current driver's license, clean driving record, and reliable insured vehicle are required.
  • Strong analytical and problem-solving skills with attention to detail are required.
  • Excellent written and verbal communication skills, including the ability to adapt coaching approaches to varied learning styles, are required.
  • A customer service mindset is required.
  • The ability to work independently and as part of a team while sharing information, completing work, and collaborating with diverse stakeholders is required.
  • Strong documentation skills and the ability to stay current on high-volume paperwork are required.
Responsibilities
  • Develop a community support plan and goals for each person on an assigned caseload.
  • Meet regularly with each person to evaluate progress and identify needed changes.
  • Develop person-centered service agreements with each person to authorize services that meet assessed needs and goals.
  • Document changes and adjustments to service agreements.
  • Authorize services to meet each person's assessed needs and goals.
  • Comply with Minnesota Health Care Programs in the delivery of waivered services such as CADI, BI, EW, and DD.
  • Work with assessors to complete MnCHOICES assessments.
